Testosterone is one of the most important hormones for men’s health. It plays a major role in energy, libido, erectile function, mood, motivation, muscle mass, bone density, red blood cell production, metabolism, and overall sense of vitality.
For many men, testosterone begins to gradually decline in the 30s and early 40s. This drop is usually slow, but over time it can become clinically meaningful. The American Urological Association, or AUA, recommends using a total testosterone level below 300 ng/dL as a reasonable cutoff to support the diagnosis of low testosterone.
The AUA also recommends that the diagnosis should be made only after two separate early-morning testosterone measurements, because testosterone levels fluctuate throughout the day.
The Endocrine Society similarly recommends diagnosing hypogonadism only in men who have both symptoms consistent with testosterone deficiency and consistently low testosterone levels. They recommend checking a morning fasting total testosterone level and confirming it with a repeat morning test.
In other words, testosterone therapy should not be based on symptoms alone or one random lab value.
A proper evaluation usually includes:
Total testosterone
Free testosterone
SHBG
LH and FSH
Estradiol
CBC/hematocrit
PSA when appropriate
Metabolic markers such as lipids, A1c, liver and kidney function
Fertility goals and prostate history review
This helps determine whether the issue is primary testicular dysfunction, pituitary signaling, lifestyle-related suppression, medication-related suppression, or another medical cause.
Testosterone Replacement Therapy Options
Once low testosterone is confirmed and treatment is appropriate, there are several ways to restore testosterone levels. The best option depends on symptoms, lifestyle, fertility goals, lab values, and medical history.
Transdermal Creams or Gels
Transdermal testosterone is applied to the skin, usually daily. This can provide a steady delivery of testosterone without needles.
It may be a good option for men who prefer a non-injectable therapy. However, patients need to be careful about skin-to-skin transfer to partners or children, and dosing consistency matters.
Testosterone Pellet Therapy
Pellet therapy involves placing small testosterone pellets under the skin, usually in the hip or gluteal area. The pellets slowly release testosterone over several months.
This can be convenient for men who do not want to apply medication daily or inject weekly. Pellet therapy may provide steady levels, although dosing must be individualized carefully.
Testosterone Injections
Injectable testosterone is one of the most common forms of testosterone replacement.
Traditionally, injections were often given once every one to two weeks. However, that approach may create peaks and valleys, where levels feel too high shortly after injection and too low before the next dose.
For many patients, smaller doses given more frequently, such as twice weekly, may help create a smoother testosterone curve and reduce highs and lows.
This can sometimes improve symptom control and minimize side effects such as mood swings, fluid retention, acne, or elevated estradiol.
Testosterone and Fertility: Why HCG May Matter
One important point that many men are not told clearly enough: testosterone replacement can reduce sperm production.
When the body receives testosterone from the outside, the brain may reduce signaling through LH and FSH. This can lower intratesticular testosterone and suppress spermatogenesis, which may lead to reduced fertility or testicular shrinkage.
For men who want to preserve fertility, testosterone therapy must be planned carefully. The Endocrine Society recommends against starting testosterone therapy in men who are actively planning fertility in the near term.
In selected patients, HCG, or human chorionic gonadotropin, may be used alongside testosterone or as an alternative strategy.
HCG acts similarly to LH and can help stimulate the testes to maintain intratesticular testosterone production. This may help reduce the fertility-suppressing effects of testosterone therapy, though it does not guarantee preserved fertility for every patient.

The AUA notes that men on testosterone therapy should have periodic follow-up testing, commonly every 6–12 months once stable.
